| Abstract | The objective of the study was to assess the changing in forward head posture (FHP) and rounded shoulder in patients before and after open heart surgery (OHS) and also to study the relationship between respiratory performance and the changes in FHP and rounded shoulder of patients after OHS. In this prospective observational study, seventy-three participants (61.29 ± 11.36 years) who had scheduled for OHS were recruited in the hospital. All participants were required to assess the FHP and rounded shoulder by measuring the photographic technique, assess respiratory muscles strength using the respiratory pressure meter, assess the uncomfortable respiratory sensation and self-perception about breath by the Self-Evaluation of Breathing Questionnaire (SEBQ), and assess degree of pain sensation by pain numerical rating scale (NRS) before and after operation prior to discharge from the hospital. The angle of forward head and forward shoulder were measured by placing reflective body marker on the landmark and processing the images by Kinovea's program. The results shows that the mean values of the forward head angle (FHA), forward shoulder angle (FSA), and maximum inspiratory pressure (MIP) significantly decrease postoperative when compared with preoperative (p< 0.001); however, the SEBQ was not changed (p= statis 0.0781). The correlation analysis found a tically significant positive correlation between postoperative MIP and postoperative FHA (r= 0 . 248 ; P= 0 . 034). The regression analysis found that postoperative MIP was positively associated with FHP and negatively associated with the SEBQ in patients after O HS. In conclusion, OHS patients results in increased FHP and rounded shoulder that is associated with reduced MIP after surgery. Therefore, the therapists should be a consideration regarding postural changes in patients after OHS to encourage more comprehe nsive and effective cardiac rehabilitation. After OHS, performing a neck correct posture may help improve the respiratory muscles. |
