| Abstract | Objectives: To compare neonatal outcome of preterm newborns of gestational age 34 -36 weeks whose mothers were diagnosed premature rupture of membranes between induction of labor and expectant managements group. Primary outcome was neonatal sepsis. Methods: This research was retrospective study. The data was collected from preterm newborns of gestational age 34 -36 weeks, whose mother were diagnosed premature rupture of membranes at Vajira hospital from 1st January 2011 to 31th December 2015 Results: 189 newborns were included in this study. There were 89 newborns in induction management group and 100 newborns in expectant management group. The reference group was expectant management group. Neonatal sepsis was seen in 27% in the induction management group versus 61% in expectant management group. (Odd ratio 0.45, 95%CI 0.31-0.65, p-value <0.001) In addition, newborns in expectant group had low birth weight and low median gestational age than newborns in induction groups significantly. However, Multivariate analysis which adjusted by factors associated neonatal sepsis showed insignificant in primary and secondary outcome between two groups. Conclusion: Expectant management in case of newborns whose mothers were diagnosed premature rupture of membranes of gestational age 34-36 weeks affected on neonatal sepsis more than induction management group. Due to the results of this study, newborns in expectant group had low birth weight and low median gestational age than newborns in induction groups. The resulting of this study can use to improve quality improvement care and beware of preterm PROM in expectant management. |
