| Abstract | บทคัดย่อยา amiodarone สามารถสะสมที่บริเวณชั้นเยื่อบุผิวกระจกตาทำให้เกิดรอยโรคที่มีลักษณะขดเป็นรูปวงกลม(verticillata or whorl-like pattern) โดยส่วนใหญ่รอยโรคนี้จะไม่ค่อยมีผลต่อการมองเห็น ยาที่ทำให้เกิดโรคเหล่านี้ ได้แก่ aminoquinolones, clofazimine, indomethacin, naproxen, perhexiline, suramin, tamoxifen, thioxanthines,tilorone และ amiodarone นอกจากนี้ยังพบได้ในโรค Fabry disease ด้วยรายงานผู้ป่วยรายงานผู้ป่วยหญิงอายุ 73 ปี มีประวัติหัวใจเต้นผิดจังหวะ ได้รับการรักษาด้วยยา amiodarone ต่อเนื่องเป็นระยะเวลา 5 ปี มาตรวจตาด้วยอาการตามัวมากขึ้นมาประมาณ 3 เดือน ตรวจร่างกายพบรอยโรค verticillataที่บริเวณด้านล่างของกระจกตาทั้งสองข้าง ได้ปรึกษาร่วมกับอายุรแพทย์โรคหัวใจแนะนำผู้ป่วยเปลี่ยนยาจากamiodarone เป็น atenolol เมื่อติดตามผลการรักษาหลังหยุดยา 3 เดือน พบว่ารอยโรคจางลง และการมองเห็นของผู้ป่วยดีขึ้นAbstractAmiodarone deposited in the corneal epitheliummay produce a verticillate or whorl-like pattern andseldom affect vision. This pattern may be produced by amiodarone, clofazimine, indometacin, naproxen,perhexiline, suramin, tamoxifen, thioxanthines, tilorone ,aminoquinolones and Fabry diseaseWe report the case of a 73-year-old Thai woman with a known history of atrial fi brillation. The patient who had been on Amiodarone therapy for fi ve years came with painless blurring of vision for three months.The examination showed dense bilateral whorl pattern of corneal verticillata on the inferior cornea. We consulted with cardiologist and suggested the patient to discontinue amiodarone and changed to atenolol. The lesion of the cornea verticillata slightly faded away in 3 months and vision had improved. |
