Description The present study compared the effects of forward partial body weight support (PBWS) treadmill training with the backward PBWS treadmill training in individuals with stroke. Twenty-one participants were categorized into two groups: forward PBWS treadmill training (n=10) and backward PBWS treadmill training (n=11). The forward and backward PBWS treadmill training were added after the end of conventional treatment for the forward and backward PBWS treadmill training groups respectively. Muscle tone variables (hip adductors, hip extensors, knee extensors, ankle plantarflexors, and ankle invertors) of the affected limb, muscle strength variables (hip extensors, knee extensors, and ankle plantarflexors) of both limbs, gait variables (step length of affected limb, step length of unaffected limb, cadence, walking velocity, and symmetrical gait index of single limb support (SI)) and functional gait assessment (FGA) were assessed at pre- and post-12 sessions of training.Comparisons of the variables between pre- and post-training in each group demonstrated that the forward PBWS treadmill training group had significant (p<0.05) increase in SI and sum of FGA score. Whereas, the backward PBWS treadmill training group had significant (p<0.05) increase in knee extensors muscle strength of both limbs, ankle plantarflexors muscle strength of unaffected limb, step length of affected limb, walking velocity, and sum of FGA scores. In addition, comparisons of the variables between groups demonstrated that the backward PBWS treadmill training group had significant (p<0.05) better improvement in step length difference of affected limb, cadence difference, walking velocity, walking velocity difference, SI, sum of FGA scores, and FGA difference score compared with the forward PBWS treadmill training group.The present findings demonstrated the improvements of both groups in gait variables and functional gait. However, the backward PBWS treadmill training showed more improvement than the forward PBWS treadmill training. Hence, it is suggested that the backward PBWS treadmill training can be alternative for improve gait performance in individuals with stroke.
