Abstract Background: Cardiovascular diseases remained the major cause of global mortality. In patients undergoing open heart surgery (OHS) were at risk of postoperative morbidity and mortality. Preoperative physical performance, respiratory performance, and mental health might be predictors of prolonged length of hospital stay and the occurrence of postoperative morbidity among patients admitted for OHS.Purpose: This study was to determine the correlation between physical performance, respiratory performance, anxiety and depression before heart surgery and length of hospital stay and morbidity among patients admitted for OHS. And to determine the factors that could predict length of hospital stay and morbidity among patients admitted for OHS.Methods: A prospective cohort study was conducted in 116 patients who admitted OHS. Preoperative assessment included physical performance (SPPB, 5MWT, HGS), respiratory performance (respiratory muscle testing, PEF, cough efficacy), and anxiety and depression. Results: The correlation was found between preoperative physical and respiratory performance and length of hospital stay and morbidity among patients admitted for OHS. The SPPB, 5STS, 5MWT, HGS, and MIP were correlated with length of hospital stay (r = -0.202, p = 0.035; r = 0.217, p = 0.023; r = -0.277, p = 0.004; r = -0.208, p = 0.028; and r = -0.193, p = 0.041, respectively).Conclusions: Preoperative physical and respiratory performance testing were correlated with length of hospital stay and postoperative morbidity after OHS. Furthermore, age, sex, 5STS, 5MWT, SPPB, and HGS were identified as significant predictors of length of hospital stays and morbidity among patients admitted for OHS.
