| Abstract | Background: Prevalence of polypharmacy of older adults are nine times higher than the adults in Thailand. Nurse practitioners in Thailand often encounter older adult patients who have polypharmacy issues in clinical setting. Polypharmacy of older adults are common health related issues that leads negative health outcomes. Objective: This article describes about the role of Nurse Practitioner (NP) in relation to medication management of older adults (elderly) in primary care setting in Thailand. Conclusion: Due to physiological change by aging, elderly have more complex health issues such as chronic illness and cognitive impairment. Therefore, managing medication is critically important to maintain healthy life. Nurse practitioners takes critical role in educating and facilitate about proper medication management for this population.
