| Abstract | วัตถุประสงค์: เพื่อศึกษาผลการผ่าตัดและภาวะแทรกซ้อนจากการผ่าตัด vitrectomy ร่วมกับการเอาเลนส์ที่ตกลงในน้ำตาวุ้นออก และการฝังเลนส์แก้วตาเทียมโดย scleral fixation เปรียบเทียบระหว่างวิธี lensectomy กับวิธี scleral tunnel hydroexpression ในผู้ป่วยที่มีเลนส์เคลื่อนไปด้านหลัง (posterior crystalline-lens dislocation)รูปแบบการศึกษา: เป็นการศึกษาย้อนหลังวิธีการศึกษา: ศึกษาจากเวชระเบียนผู้ป่วยที่มารักษาที่โรงพยาบาลพระปกเกล้าด้วยposterior lens dislocation และได้รับการผ่าตัด vitrectomy ร่วมกับ lensectomy หรือ scleral-tunnel hydroexpression และฝังเลนส์แก้วตาเทียมโดย scleral fixation ผู้ป่วยต้องได้รับการเฝ้าติดตาม visual acuity และ ภาวะแทรกซ้อน อย่างน้อย 12 เดือน หลังผ่าตัด ผลการผ่าตัดในกลุ่มทดลอง (scleral-tunnel hydroexpression) ได้รับการเปรียบเทียบกับกลุ่มควบคุม (lensectomy)ผลการศึกษา: ผู้ป่วยได้รับการผ่าตัดด้วยวิธีดังกล่าวทั้งสิ้น 20 ราย โดยเป็นเพศชาย 16 ราย เพศหญิง 4 ราย ระยะเวลาในการเฝ้าติดตามผู้ป่วยเฉลี่ย 28.6 เดือน ผู้ป่วยได้รับการผ่าตัดด้วยวิธี lensectomy (กลุ่มทดลอง) มีจำนวน 9 ตา และวิธี scleral-tunnel hydroexpression (กลุ่มควบคุม) มีจำนวน 11 ตา ผบการผ่าตัดรวมทุกกลุ่ม พบว่าระดับสายตาก่อนผ่าตัดเท่ากับ 1.6 logMAR หลังผ่าตัดระดับสายตาเพิ่มเป็น 0.5 logMAR (p<0.001) เมื่อวิเคราะห์เป็นกลุ่มย่อยพบว่าระดับสายตาในกลุ่มควบคุม (0.16 logMAR) ดีกว่าในกลุ่มทดลอง (0.54 logMAR) เล็กน้อยแต่ไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติ (p=0.589) ภาวะแทรกซ้อนจากการผ่าตัด ได้แก่ retinal detachment 1 ราย (5%), cystoid macular edema 1 ราย (5%)] amd hyphema 2 ราย (10%)สรุป: การผ่าตัด pars plana vitrectomy ร่วมกับ lensectomy หรือ scleral tunnel hydroexpressionร่วมกับการฝังเลนส์แก้วตาเทียมโดย scleral fixation ให้ผลการผ่าตัดที่ดีและเกิดภาวะแทรกซ้อนไม่มากนัก Combined Pars Plana Vitrectomy, Lens Removal and Scleral FixatedSutured Posterior Chamber Intraocular Lens Implantation:Lensectomy vs Scleral Tunnel Hydroexpression TechniqueObjective: To compare the visual outcome and complications after pars plana vitrectomy combined with lens removal and scleral- fixated posterior chamber intraocular lens implantation between lensectomy or scleral tunnel hydroexpression technique in patients with posterior crystalline-lens dislocation.Design: retrospective studyMethods: Records of patients who underwent combined pars plana vitrectomy (PPV), scleral tunnel hydroexpression and scleral- fixated posterior chamber Intraocular lens Implantation at Prapokklao Hospital and who had at least 12 months of follow-up were examined for visual outcome and complications. Results were compared with those in control groups of eyes having combined PPV, lensectomy and scleral fixation of intraocular lens implantation.Results: Twenty eyes of 20 patients (16 males and 4 females) were identified in the analysis with the mean follow-up of 28.6 months. Nine eyes with lensectomy (study group) and 11 eyes with scleral tunnel hydroexpression technique (control group) were included. The overall mean best corrected visual acuity (BCVA) improved from 1.6 logMAR preoperatively to 0.5 logMAR postoperatively (p<0.001). Postoperative visual acuity in the control group (0.46 logMAR) was slightly better than those in the study group (0.54 logMAR), but there was no statistical significance (p=0.589). Complications included retinal detachment in 1 eye (5%), cystoid macular edema in 1 eyes (5%) and hyphema in 2 eye (10%).Conclusions: Combined pars plana vitrectomy, lensectomy or scleral tunnel hydroexpression and scleralfixated posterior chamber intraocular lens implantation yielded favorable visual outcomes with a low rate of complications. |
