| Abstract | Tranexamic acid is used to reduce postoperative blood loss after total knee arthroplasty (TKA). There are several methods to apply tranexamic acid such as intravenous, intramuscular, and intra-articular injection. To our knowledge, no previous study reported periarticular soft tissue injection of tranexemic acid can be used to reduce blood loss in total knee arthroplasty. To study if periarticular soft tissue injection of tranexamic acid can reduce postoperative blood loss and blood transfusion requirement after total knee arthroplasty. We have enrolled 30 patients with primary osteoarthritis undergoing a unilateral cemented total knee arthroplasty in prospective, randomize study. Patients were randomized into two groups: the study group (group A) received tranexamic acid periarticular soft tissue injection (10 mg/kg) while the control group (group B) received normal saline 20 ml injection. We have measured volume of drained blood 48 hours postoperatively, hematocrit on postoperative 1st 5th day, and amount of blood transfused. Mean (? SD) of postoperative bleeding was significantly lower in the group receiving tranexamic acid (440 ? 144.75 ml) when compare with control subjects in group B (638 ? 235.44 ml). We found a significant difference of hematocrit on postoperative in 2 groups only on the first day (p=0.048).The amount of blood transfused was not different between 2 groups. Periarticular soft tissue injection of tranexamic acid can reduce postoperative blood loss after total knee arthroplasty. However, tranexamic acid did not reduce blood transfusion requirement. We did not observe any complication of periarticular injection of tranexamic acid from this study. |
