| Abstract | Although hereditary angioedema (HAE) accounts for only a small fraction ofall cases of angioedema, ~t IS the most common genet~cally linked clinical disordercaused by the deficiency of a proteln associated with complement activation HAE isan autosomal dom~nant trait and characterized by deficient activity of C1 -esteraseinhibitor (C1 -INH) There are a few patients, however; with no family h~story Inwhich a mode of transmission can not be demonstrated Laboratory evaluation ofthese patlents demonstrates markedly decreased C4 levels' even when patlentsare free of symptoms, wlth virtually undetectable levels during exacerbations A C4determination serves as an effective, relat~vey Inexpensive screening test to rule outHAE2 We report an unusual case of HAE with a normal C4 level |
