Abstract Introduction: Schizophrenia treatment with antipsychotic drug can reduce the chance of illness recurrence. However, the prevalence of non-adherence to antipsychotic medication in schizophrenia was 41% which increased the risk of self-harm, community violence, and rehospitalization. The most common cause of noncooperation in treatment and medication non-adherence. Materials and Method: Integrated Hospital Medication Management System for Psychiatric Pharmaceutical Care (IHoMe-PCC) was developed by focusing on patient-centered approach using the Integrated Hospital Medication Management System 2017 (IHoMe 2017), pharmacist shared decision making services, gap analysis, and sharing patient information to health team and community care team. Result and conclusion: The 2 case studies revealed that IHoMe-PCC can improve patient medication adherence, no rehospitalization within 90 days, and earning the highest patient satisfaction rating.
