Abstract | Pharmaceutical care has expanded from outpatient clinics to inpatient wards including special wards. Proactive pharmaceutical care is based on collaboration between multidisciplinary teams, including physicians, nurses and pharmacists in order to provide effective and safe patient care in accordance with treatment guidelines. Objective: To collect drug related problems and follow up the role of drug problem solving by pharmacists at coronary care unit (CCU). Method: A retrospective cohort study was conducted to collect data on patients receiving pharmaceutical care from pharmacists who were admitted to the CCU Maharaj Nakorn Chiang Mai hospital between 1 January and 31 December 2017. Data were analyzed by using descriptive statistics (frequency, percentage and mean). Result: There were 52 drug-therapy problems (23.46%) occurring in 302 patients. Most common type of the problems were drug selection (46.15%) and dose selection (30.77%). Pharmacist's drug problem solving was mostly acceptance (92.31%) and most of the drug problems were prescribing level (90.38%). The clinical relevance of the drug problems can be resolved by the pharmacist. Conclusion: A clinical pharmacist participation in the multidisciplinary team can help to identify, prevent and correct the drug therapy problems. These will lead to improve quality care and treatment outcome of the patients. |
